New To Physical Therapy Services?

Here is some info for first time patients:

So very glad you're here and we are honored that you selected PTS to help you through your recovery!


Here are a few items to know, things to bring, and what to expect at your first visit.


Please arrive 15 to 20 minutes before your first appointment. We understand that this is an inconvenience, but in order to help your Physical Therapist get to know your injury or illness there are health history forms that need to be filled out, plus obtain your consent to have PTS treat your condition, and we need your consent to bill your insurance - all forms need to be filled out by you. And there are a lot of them. Our Physical Therapists would like to spend more time with you, so arriving 15 to 20 minutes earlier than your scheduled appointment is greatly appreciated.


Please bring a valid photo ID. Due to HIPAA, we are required to request from each new patient or returning patient a valid photo . This is to protect you from identity theft. We are also required to make a photo copy and include this in your medical records.


Also, please bring a current health insurance card. We will need a copy of the insurance card you provided over the phone to contact them for any benefit, eligibility, and claim issues/concerns.


A list of your prescription and over the counter medications. If you are a patient with Medicare, Medicare Replacement, or Medicare Supplements, Medicare requires us to obtain a current list of all your medications containing: Medication name, dose, and the frequency you take the medication in a day. Sometimes your physician will send over your medication list along with the Physical Therapy referral, which is nice, but in some cases, the list is not provided. If the list is not provided by your physician we will ask you to bring your over the counter medication as well as a list of your prescription medication too.


Also for patients having Medicare, Medicare Replacement, or Medicare Supplements, please know that Medicare requires us to take your height and weight too. Please let us know if you would like to have these taken in the privacy of a closed room and we will certainly accommodate.


During your first visit, also known as Initial Evaluation, to understand the gravity of your injury, your Physical Therapist will perform a series of tests to determine the limit of your ability to move or not move, as well as your pain level. In the course of these tests, your Physical Therapist will determine what best and appropriate treatment plan for you to aide in your recovery back to normal.


PTS prides itself in knowing that the Physical Therapists (PT) work synchronously with the Physical Therapist Assistant (PTA) to make sure you get the best care we can provide to you so that you can recover in the shortest possible time. It is quite possible that while you are being treated, you might be treated by either a PT and/or a PTA.


At the end of your first visit your Physical Therapist will let you know the frequency of your visits per week. Your Physical Therapist will walk with you back to the counter where you Checked-In, will speak to the Support Staff to let them know what the appropriate visit frequency, and then you can work with the Support Staff to schedule your future appointments.


At some point during your first visit the Support Staff will inform you that you will be receiving a text reminder the afternoon or evening before your next scheduled appointment.


Also, you will be informed whether or not that your insurance will require a Prior Authorization (PA) before scheduling your next appointment. Some insurance companies can take anywhere between 3 - 10 days to determine whether or not you will be authorized to receive additional PT visits. We strongly encourage you to call your insurance company and speak to the Customer Service representatives to help expedite this process.


Please know that if you cancel your appointment within 24 hours or one business day before your next scheduled appointment, you might be subject to a Cancellation Fee. For example, say today is Friday and your next scheduled appointment is Monday, but you were ill and could not make your Monday appointment, just give us a ring and leave a message. We will be happy to cancel your Monday appointment for a valid reason and you won't be charged a Cancellation Fee. So please call the clinic well in advance if you know that you cannot attend your next scheduled visit to re-schedule your next appointment.
